Annex IV — Financial identification form

Financial identification:
The financial identification form needs to be signed by the tenderer and supported by certification (self-declarations excluded) by the bank; this is in the form of one of the following:
1) either the bank stamp and date in the form bottom left box,
2) or alternatively it can be substituted by attaching a copy of a bank statement (with the private (or felt so) information covered up and hidden).

Question 1
I am currently preparing a submission in response to the call for tender in subject. I had a query regarding part of task 3 within the tender specification. The task requires “broad consultation with experts and already existing European network(s) on Lyme borreliosis to validate the guidance…”. The tender specifications also state that all expenses should be included in the price of the submission, and that no expenses incurred during the undertaking of the assignment shall be reimbursed. Can I therefore assume that any honoraria costs that may need to be paid to consulting experts would not be reimbursed, and therefore the cost of this should either be incorporated within the project cost, or only experts who are willing to participate without payment should be included?
Answer
All costs related to the project, also honoraria costs or cost related to the consultation of experts to validate the guidance (task 3 of the tender), need to be included in the total costs of the project proposal.

 

Question 2
We are currently in the process of developing this tender, however wanted to clarify the budget before submitting. The tender specification mentions that the maximum budget for this project is 60,000.00 Euro. Will any submissions received that are outside of this budget be automatically rejected (i.e. is the budget an ‘exclusion factor’)?
Answer
The maximum budget available for this project is 60,000.00 Euro. Submissions above the mentioned budget cannot be awarded.
